Hospice Triage RN PT, 3:30p-11p, Rotating Sat & Sun

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage services to hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Collaborate with on-call teams to ensure coordinated care and resource allocation.

• Accurately record all communications and interventions made.

• Maintain a calm and professional demeanor while addressing urgent calls.

• Offer essential after-hours support and manage care for hospice patients and their families.

⛳️ Requirements

• Must possess an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documenting calls.

• Capability to type documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask effectively while receiving calls.

• Strong communication and critical thinking abilities are essential.

• Must be able to work independently in a remote work environment.

• Consistent attendance, proactive communication, and stable internet connectivity during scheduled shifts are required.

• Starting pay of $25 per hour, with opportunities to increase to $28 per hour within the first 6 months of employment.

• $2 shift differential for overnight shifts.

• $2 shift differential for weekend shifts (Friday evening, Saturday & Sunday).
