Hospice Triage RN, PT 3:30p-11p, Rotating Sat & Sun 7:30a-4p CST

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and determine suitable next steps.

• Collaborate with on-call teams to coordinate care and resources efficiently.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Deliver essential after-hours support and facilitate timely interventions for hospice patients receiving services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Ability to document notes quickly in a fast-paced environment.

• Capability to manage stress and multitask effectively while receiving calls.

• Strong communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Consistent attendance, proactive communication, and dependable internet connectivity during all scheduled shifts.

• Availability for rotating Saturday and Sunday shifts, as specified in the schedule.
