Hospice Triage RN, Part-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions to determine the appropriate course of action.

• Collaborate with on-call teams to ensure coordinated care and resource availability.

• Accurately document all communications and interventions performed.

• Maintain a composed and professional demeanor while managing urgent calls.

• Provide essential after-hours support and coordinate timely interventions for hospice patients receiving care.

⛳️ Requirements

• Active multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Ability to type documentation and notes efficiently in a fast-paced environment.

• Capability to manage stress and multitask effectively while handling calls.

• Strong communication skills and critical thinking abilities.

• Ability to work independently in a remote setting.

• Consistent attendance, proactive communication, and reliable internet connectivity during all scheduled shifts.
