Hospice Triage RN, Part-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and establish suitable next steps.

• Work collaboratively with on-call teams to facilitate care and resources.

• Ensure precise documentation of all communications and interventions.

• Maintain a composed and professional demeanor when managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and families.

• Coordinate timely interventions and care for patients enrolled in hospice services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for documenting calls.

• Ability to efficiently type documentation and notes in a fast-paced setting.

• Capable of managing stress and multitasking while receiving calls.

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Reliable attendance, proactive communication, and stable internet connectivity during scheduled shifts.

• Availability from 3:30 p.m. to 11:00 p.m. CST, including rotating shifts on Saturdays and Sundays.
