Hospice Triage RN, Part-Time

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Conduct telephone triage for hospice patients and their families

• Evaluate patient conditions and establish appropriate subsequent actions

• Collaborate with on-call teams to facilitate care and allocate resources

• Accurately record all communications and interventions

• Maintain a composed and professional attitude while managing urgent calls

• Offer essential after-hours support for the needs of hospice patients and their families

• Coordinate prompt interventions and care for patients receiving hospice services

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficiency with technology and electronic medical records (EMR) for documentation purposes

• Capability to document and note in a fast-paced setting

• Capacity to manage stress and multitask while receiving calls

• Excellent communication and critical thinking abilities

• Ability to work autonomously in a remote setting

• Consistent attendance, proactive communication, and reliable internet access during all scheduled shifts
