Hospice Triage RN, Part-Time

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and identify appropriate subsequent actions.

• Work collaboratively with on-call teams to organize care and resources.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and families via phone.

• Coordinate prompt interventions and care for patients utilizing hospice services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documenting calls.

• Capability to type documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask effectively while taking calls.

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Reliable attendance, active communication, and dependable internet access during all scheduled shifts.
