Hospice Triage RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Offer after-hours triage, clinical evaluation, and interventions over the phone.

• Assist patients and their families with managing symptoms and addressing end-of-life care requirements.

• Facilitate care coordination by collaborating with physicians, facilities, and internal teams.

• Organize visits and coordinate services with vendors as necessary.

• Provide clear and compassionate education and support to families.

• Accurately record all interactions, care plans, and outcomes in the EMR.

• Review team documentation for precision and compliance.

• Collaborate with Telecare staff to ensure favorable patient outcomes.

⛳️ Requirements

• Active RN license with no current or pending restrictions.

• Experience in hospice care.

• Familiarity with Home Care Home Base (HCHB).

• Strong clinical judgment and critical thinking skills.

• Exceptional communication and phone-based patient care abilities.

• Capability to multitask in a dynamic, remote environment.

• Excellent customer service and conflict resolution skills.
