Hospice Triage RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver after-hours triage, clinical evaluations, and interventions via telephone.

• Assist patients and their families with managing symptoms and addressing end-of-life care requirements.

• Facilitate care by working collaboratively with physicians, facilities, and internal teams.

• Organize visits and coordinate services with vendors as necessary.

• Provide clear and compassionate education and support to families.

• Meticulously document all interactions, care plans, and outcomes in the EMR.

• Review team documentation for accuracy and regulatory compliance.

• Collaborate with Telecare staff to ensure favorable patient outcomes.

⛳️ Requirements

• Current RN license (with no ongoing or pending restrictions).

• Experience in hospice care.

• Familiarity with Home Care Home Base (HCHB).

• Strong clinical judgment and critical thinking skills.

• Exceptional communication and phone-based patient care abilities.

• Capability to multitask in a dynamic, remote setting.

• Strong customer service and conflict resolution capabilities.
