Hospice Intake Nurse Coordinator

This is a fully remote position, open to applicants in Florida, +8 more states.

📋 Description

• Act as the main backup Intake Nurse for all Ennoble Care markets, ensuring coverage during PTO, absences, and staffing shortages.

• Support market Intake Nurses with additional referrals and heightened workloads during peak periods.

• Receive, review, and manage referral and intake information in compliance with company policies and regulatory standards.

• Accurately complete referral and intake forms and direct them appropriately for admission approval.

• Confirm Medicare coverage and verify benefit period details using appropriate systems and working files.

• Assist in organizing and scheduling clinicians with the necessary skills to fill cases, entering scheduling data into the EMR system.

• Provide standardized education and training to newly onboarded Intake staff across all markets.

• Create and maintain training materials, reference guides, and standard operating procedures related to the intake process.

• Act as a subject matter expert on best practices for intake, regulatory requirements, and EMR system capabilities.

• Collaborate with the Compliance Department to identify opportunities for process improvements and support quality assurance initiatives pertaining to intake operations.

• Offer continuous mentorship and coaching to market Intake Nurses as required.

• Maintain effective and timely communication with market leadership, clinical teams, and the Compliance Department.

• Engage in team meetings, training sessions, and compliance-related activities as necessary.

• Perform additional duties as assigned.

• Occasional travel may be necessary for in-person training sessions or company meetings.

⛳️ Requirements

• At least 2 years of experience in hospice or home health intake.

• A minimum of 3 years of experience working with EMR systems.

• Proficient computer skills with the capability to navigate various systems and maintain precise electronic records.

• Outstanding verbal and written communication abilities.

• Proven capability to work independently in a remote setting with strong organizational and time-management skills.

• Active LPN license in good standing (multi-state or Compact license preferred).

• Experience in training, mentoring, or onboarding clinical or administrative personnel.

• Knowledge of Medicare, Medicaid, and commercial insurance verification processes.

• Experience working across multiple markets or regions within a home health or hospice organization.

• Reliable high-speed internet connection and a dedicated, private workspace.
