Homeless Services, Coordinated Entry System Coordinator

This is a fully remote position, open to applicants in California.

📋 Description

• Execute the CoC CES policies, procedures, and requirements, which includes CES assessment tools, prioritization policies, referral processes and workflows, HMIS technologies, project-level eligibility, and matching protocols.

• Oversee and manage coordinated entry procedures to minimize unit vacancies, accelerate referrals from CES to housing, and enhance move-in processes.

• Collect and analyze data regarding household strengths, barriers, service offerings, and eligibility criteria.

• Implement standardized CES guidelines, which cover housing applications, service coordination, and data tracking.

• Facilitate cross-functional, interagency, and system-wide collaboration to pinpoint workflow challenges and devise solutions.

• Create training materials, compliance training, operational forms/guides, FAQs, and toolkits.

• Convert policy requirements and technical documentation into actionable CES processes.

• Coach and guide service providers and CES partners on protocols and procedures.

• Identify process enhancements to promote timely referrals to eligible and prioritized housing options.

• Offer field-based support, staffing augmentation, direct training, technical assistance, capacity building, and quality assurance monitoring.

• Cultivate relationships with CES providers, access sites, affordable housing providers, behavioral health partners, and substance use treatment facilities.

• Analyze HMIS and CES data to uncover trends and inequities, recommending policy and programmatic enhancements.

• Maintain secure communication channels to uphold client privacy and confidentiality.

⛳️ Requirements

• Bachelor's degree in areas related to Human Services, Public Policy and Administration, Business, or Social Welfare; equivalent experience may be considered.

• A minimum of two years of relevant experience with the Continuum of Care program and HUD Coordinated Entry programs.

• Familiarity with the Continuum of Care program and HUD Coordinated Entry Requirements as outlined in the CoC Program interim rule and HUD Notice CPD-17-01.

• Knowledge of HUD resources and guidebooks concerning CES topics.

• Exceptional verbal, interpersonal, and written communication skills.

• Capability to elucidate complex concepts to non-technical stakeholders.

• Strong organizational, facilitation, conflict resolution, and problem-solving abilities.

• Aptitude for prioritizing and managing multiple tasks simultaneously in a dynamic environment while adhering to strict deadlines.

• Experience in managing sensitive information and maintaining confidentiality.

• Understanding of Housing First, trauma-informed care, person-centered service delivery, and principles of racial equity.

• Proven experience in facilitating case conferencing or multidisciplinary coordination meetings, managing conflict, and achieving consensus among multiple stakeholders.
