This is a fully remote position, open to applicants in Virginia.
• Conduct in-home infusion therapy visits in alignment with the established care plan.
• Prepare clinical and nursing documentation while recording patient interventions in both clinical and non-clinical records.
• Act as a clinical resource and collaborate closely with pharmacy departments.
• Stay informed about drug side effects and interactions.
• Provide both telephone and face-to-face patient education on therapy protocols, drug side effects, and home infusion procedures.
• Ensure patients have a clear understanding of therapy plans, medications, and the significance of adherence.
• Serve as a liaison among patients, families, and the pharmacy.
• Conduct thorough initial and periodic head-to-toe assessments of patients.
• Initiate preventive and rehabilitative care as needed.
• Assess patient needs using evaluation data and insights from the agency team, physician, patient, and family.
• Manage expectations of patients and families regarding services, outcomes, and discharge goals.
• Establish primary and secondary diagnoses based on assessments and home healthcare focus.
• Develop care plans incorporating skilled interventions, medical supplies, and required services.
• Evaluate patient progress alongside team members and update care plans as necessary.
• Perform additional duties as assigned.
• Graduate from an accredited nursing program.
• Current RN state license.
• A minimum of 2 years of experience in home infusion and infusion centers.
• At least 2 years of experience with IV Immunoglobulins and IV Biologics.
• CRNI certification is preferred.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.
• Strong interpersonal, time management, and organizational abilities.
• Excellent written and verbal communication skills.
• Ability to consistently maintain accurate records.
• Capability to direct information to the relevant healthcare provider.
• Proficient computer skills, including word processing, internet navigation, and email usage.
• Current CPR and BLS certifications from the Red Cross or AHA.
• Valid driver’s license, dependable transportation, and automobile insurance.
• Ability to lift up to 20 pounds on occasion.
• Ability to maintain constant communication with patients and team members via telephone and text messaging.
• Flexible working hours.
