This is a fully remote position, open to applicants in Arizona.
• Conduct in-home infusion therapy sessions in accordance with the established care plan.
• Prepare clinical and nursing documentation while recording patient interventions in both clinical and non-clinical files.
• Serve as a clinical resource and work collaboratively with pharmacy departments.
• Stay informed about drug side effects and interactions.
• Provide both telephone and face-to-face education on therapy protocols, drug side effects, and home infusion procedures.
• Ensure that patients comprehend therapy plans, medications, and adherence requirements.
• Act as a communication link between patients, their families, and the pharmacy.
• Conduct thorough initial and ongoing head-to-toe assessments of patients.
• Initiate preventative and rehabilitative care as necessary.
• Identify patient needs using assessment data and feedback from the care team, physician, patient, and family.
• Manage expectations of patients and families concerning services, outcomes, and discharge objectives.
• Establish primary and secondary diagnoses based on patient evaluations and the focus of home health care.
• Create care plans that include skilled interventions, medical supplies, and required services.
• Assess patient progress and modify care plans as required.
• Carry out additional duties as assigned.
• Graduate from an accredited nursing program.
• Active RN state license.
• A minimum of 2 years of experience in home infusion and infusion centers.
• At least 2 years of experience with IV Immunoglobulins and IV Biologics.
• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients.
• CRNI certification is preferred.
• Strong interpersonal, time management, and organizational capabilities.
• Excellent written and verbal communication skills.
• Ability to maintain accurate records consistently.
• Capability to direct information to the appropriate healthcare provider.
• Proficient in computer applications, including word processing and effective use of the internet and email.
• Current CPR and BLS certifications from the Red Cross and AHA.
• Valid driver's license.
• Reliable transportation and automobile insurance.
• Ability to lift up to 20 pounds occasionally.
• Flexible working hours.
• Per diem scheduling.
• Independent contractor arrangement.
