Home Infusion Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and deliver intermediate direct patient care in patients’ homes or alternative infusion suites following a physician’s care plan.

• Initiate, design, implement, and assess intermediate nursing care plans.

• Administer treatments, provide medications, and educate patients and their families or caregivers.

• Adjust treatment plans based on evolving patient conditions or physician directives.

• Assess patient requirements and physical condition during skilled visits.

• Obtain and clarify physician orders, promptly notifying physicians of significant changes in patient conditions.

• Offer training and mentorship to Infusion Nurse I staff as required.

• Instruct patients and families on how to independently manage prescribed therapies and care needs.

• Communicate with clinical teams, medical professionals, patients, and caregivers to ensure uninterrupted care.

• Prepare and submit real-time clinical documentation.

• Coordinate discharge planning and create discharge summaries.

• Adhere to agency policies, professional standards, regulations, and accreditation requirements.

• Utilize computers and automation technologies to validate orders, document care, and maintain assigned visit schedules.

• Respond to patient requests and initiate emergency protocols when necessary.

• Protect confidential patient and proprietary information in compliance with HIPAA regulations.

• Explain Option Care services and associated financial responsibilities to patients.

• Complete necessary training and continuing education to uphold competency and licensure.

• Oversee and coordinate the work of paraprofessionals and assume the role of Supervisor when applicable.

• Engage in multidisciplinary conferences, precepting, training, mentoring, and nursing on-call duties.

• Insert PICC/Midlines and troubleshoot or resolve issues with obstructed central-line catheters as needed.

• Monitor PICC/Midlines and educate patients on infection prevention and regular care.

⛳️ Requirements

• Current and unrestricted RN license in the state of practice.

• At least 1 year of nursing experience.

• Valid CPR certification is required.

• Proven competency in patient care standards essential for the safe delivery of services and infusion skill sets relevant to agency programs and service needs.

• Advanced certifications and training as required.

• Valid licensure to operate a motor vehicle in the state of practice.

• Access to a vehicle for business-related travel with proof of liability insurance.

• Basic understanding of computer operating systems and software applications.

• Ability to lift up to 50 pounds, with a maximum lifting capacity of 75 pounds.

• Willingness to travel up to 100% of the time for work purposes.

• Bachelor of Science in Nursing from an accredited nursing school is preferred.

• 1-2 years of prior infusion nursing experience is preferred.
