Home Health Licensed Practical Nurse

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Deliver proficient nursing care to patients in their residences under the supervision of a Registered Nurse (RN).

• Execute customized care plans in collaboration with patients, families, and the healthcare team.

• Inform patients and their families about disease management, treatment choices, and self-care practices.

• Keep precise records of patient care, documenting visits, interventions, and outcomes.

• Promptly communicate with patients, physicians, clinical managers, and healthcare team members regarding needs and changes in status.

• Engage in care coordination and discharge planning.

• Report directly to the Clinical Manager.

• Aid in patients' recovery, rehabilitation, and autonomy within a home health environment.

⛳️ Requirements

• Active nursing license in the state of practice.

• Valid driver's license.

• Auto insurance coverage.

• Dependable transportation.

• Current CPR certification.

• A minimum of two years of experience as an LPN/LVN in a clinical environment.

• Must carry personal vehicle liability insurance that meets state minimums or specified Humana limits, whichever is greater.

• Required to complete tuberculosis screening if selected for a patient-facing position.
