Home Care Scheduling Specialist

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Organize home health and hospice appointments as directed by physicians

• Arrange starts of care, initial assessments, resumptions of care, and routine visits

• Schedule multiple disciplines across three counties and ten territories throughout Central Oregon

• Coordinate visits by assessing territories and caseloads to ensure efficiency and adherence to regulations

• Accurately input appointments into the scheduling system and relay important information to staff

• Assist Intake and triage by overseeing faxes, inbasket messages, emails, referrals, labs, orders, and documentation

• Communicate any staffing changes, pinpoint scheduling issues, and implement corrective measures

• Monitor home health patients with inpatient status and facilitate timely resumption of care post-discharge

• Deliver exceptional customer service and ensure follow-through on resolutions or referrals

• Navigate EMRs and platforms such as Teams, Excel, OneNote, Company Mileage, and RightFax

• Generate and update departmental data reports

• Screen for safety issues and uphold a safe environment for caregivers, patients, and visitors

• Maintain professionalism, confidentiality, regulatory compliance, and ethical standards

• Contribute to Lean continuous improvement initiatives and undertake additional assigned responsibilities

⛳️ Requirements

• High school diploma or GED required

• Previous experience in customer service within a high-volume, fast-paced setting

• Prior experience in an administrative or clerical role

• Familiarity with medical terminology and procedures

• A minimum of two years of general office experience, preferably in a hospital setting

• Proficient phone skills, computer literacy, and strong interpersonal abilities

• Capability to wear appropriate Personal Protective Equipment (PPE)

• Excellent teamwork and collaboration skills

• Flexibility to work a variable schedule, including two days a week with rotating weekends and holidays
