This is a fully remote position, open to applicants in Oregon.
• Arrange home health and hospice appointments as directed by physicians.
• Organize starts of care, initial assessments, resumptions of care, and routine visits.
• Schedule various disciplines across three counties and ten territories in Central Oregon.
• Coordinate visits among disciplines by evaluating territory and caseload for efficiency and adherence to regulatory standards.
• Accurately input appointments into the scheduling system and relay essential information to the relevant staff.
• Assist Intake and triage by handling faxes, inbasket items, emails, referrals, labs, add orders, information requests, and documentation scanning.
• Inform the patient care team of staffing changes, identify issues, implement corrective actions, and communicate modifications.
• Oversee Home Health patients with inpatient status and facilitate transfers and timely resumption of care following discharge.
• Deliver customer service and ensure follow-through to resolution or referral.
• Navigate electronic medical records (EMRs) and platforms such as Teams, Excel, OneNote, Company Mileage, and RightFax; generate and update departmental data reports.
• Identify potential safety concerns and report them to the appropriate personnel.
• Collaborate with the scheduling team, Intake team, patients, families, clinicians, leadership, and other members of the patient care team.
• Uphold professionalism, confidentiality, regulatory compliance, ethical conduct, and organizational integrity.
• Support the organization's vision, mission, values, and Lean continuous improvement principles.
• Carry out additional duties of similar complexity as assigned.
• High school diploma or GED.
• Previous experience in customer service within a high-volume, fast-paced setting.
• Prior experience in an administrative or clerical role.
• Familiarity with medical terminology and procedures.
• Two years of general office experience, ideally in a hospital setting.
• Proficiency in phone communication, computer skills, and interpersonal interactions.
• Capability to wear appropriate Personal Protective Equipment (PPE).
• Ability to communicate clearly and audibly and hear normal speech levels.
• Capacity to stand, sit, walk, lift 1–10 lbs, and operate a keyboard.
• Ability to bend, climb stairs, reach overhead, carry 1–10 lbs, push/pull 1–10 lbs, and grasp/squeeze.
• Capability to stoop/kneel/crouch, lift 11–25 lbs, carry 11–25 lbs, push/pull 11–25 lbs, and operate a motor vehicle.
• Preferred: Associate degree or higher; additional college coursework in information systems, office management, and medical technology.
• Preferred: 1–2 years of scheduling experience in a fast-paced environment.
