Health Data Specialist

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Collect, oversee, and report on outcome data and activity monitoring pertinent to the mandated OhioRISE CME activities.

• Supervise a substantial population of individuals and families/caregivers who receive support through care coordination and management efforts.

• Ensure that all necessary activities and reporting are completed for individuals within the designated population pool.

• Collaborate with assigned care coordinators to identify and address any gaps.

• Acquire or exchange essential records or documentation with staff from provider or community organizations.

• Primarily communicate with care coordination teams via telephone, video conferencing, and electronic communication.

⛳️ Requirements

• High School diploma or GED is mandatory.

• One to three years of experience in handling health data, data analytics, health outcomes reporting, or using data reporting tools.

• A valid Ohio Driver’s License issued by the Ohio Bureau of Motor Vehicles.

• Proof of automobile insurance is required.

• No more than 5 points on the driving license.

• Successful completion of a background check, drug screening, and health screening, as relevant to the position.
