Head of Patient Guides

This is a fully remote position, open to applicants in United States.

📋 Description

• Manage the patient guide sector for up to 10 bundles of carepods, catering to approximately 4,500 high-acuity patients.

• Recruit, onboard, and supervise Directors of Patient Guides.

• Develop systems that promote consistency, accountability, and clinical leadership among patient guide teams.

• Mentor Directors of Patient Guides in clinical supervision, personnel management, productivity, and change management.

• Assist with leadership coverage planning during PTO, leave, and turnover periods.

• Ensure the delivery of consistent, high-quality patient care across all carepods.

• Uphold fidelity to the Flourish integrated care model at scale.

• Aid in managing clinical escalations and complex patient care scenarios.

• Establish and maintain expectations and protocols for guide in-home visits.

• Identify operational or clinical deficiencies and implement scalable enhancements.

• Collaborate with leaders in psychiatry, therapy, family guides, operations, recruiting, and people operations.

• Support the effectiveness of integrated carepods, organizational scaling efforts, and workforce planning.

• Drive change management, team culture, motivation, and employee engagement.

• Provide direct patient care for 2-5 hours weekly.

• Reports to the VP of Integrated Patient Care.

⛳️ Requirements

• Over 10 years of relevant experience in patient coaching.

• More than 10 years of people management experience overseeing full-time direct reports.

• At least 2 years of experience leading multi-state behavioral health teams.

• Minimum of 2 years managing through multiple layers of leadership.

• 8+ years of experience in startup environments.

• Availability to work one evening per week.

• Must be at least 18 years old.

• Required to provide direct patient care for 2-5 hours per week.

• Ability to work remotely and attend in-person events at least 2 times per year.
