This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and manage a caseload of applicants or caregivers through the HCBA waiver enrollment process.

• Provide continuous case management services throughout the enrollment phase.

• Monitor, document, and maintain progress notes and case records in accordance with agency policies and timelines.

• Accurately record patient interventions and their responses.

• Keep updated timekeeping and scheduling records.

• Collaborate with the RN on the Intake Team for effective case management.

• Report any signs of abuse or neglect to the relevant agencies.

• Confirm clients' active Medi-Cal eligibility on a monthly basis and assist with applications as necessary.

• Prepare Institutional Deeming letters.

• Organize client visits and RN appointments.

• Document case notes within 24 hours and ensure contact information is updated in MedCompass and NetSmart.

• Conduct acuity assessments and other necessary evaluations.

• Develop participant goals tailored to assessed needs, circumstances, and preferences.

• Identify external services for documentation on the Plan of Treatment (POT).

• Offer information, education, counseling, advocacy, and support in accessing waiver services.

• Assist patients and their families in navigating family and community agencies.

• Establish care coordination schedules based on participant needs and acuity levels.

• Safeguard patient and agency confidentiality in compliance with HIPAA.

• Attend case conferences, staff meetings, and agency training sessions.

• Engage in continuing education and uphold professional credentials.

• Respond to supervisor communications within a 24-hour timeframe.

• Carry out other assigned responsibilities.


⛳️ Requirements

• Master's degree in Social Work or a bachelor's degree in a related field.

• At least two years of experience in providing direct mental health or intensive case management services.

• Experience working with clients who have mental illness or chronic health conditions.

• Strong knowledge of managed care organizations, Medicare, and Medi-Cal.

• Exceptional interpersonal skills and a sensitivity to culturally diverse populations.

• Highly organized and capable of adhering to documentation and reporting standards.

• Ability to maintain professional boundaries while managing time efficiently.

• Current CPR certification is required.

• Must maintain an active license in the State of California.

• Ensure all credentials are kept current.

• Capable of performing necessary computer, telephone, keyboard, walking, lifting, pushing, pulling, and driving tasks.

• Able to lift and push/pull weights up to 25 pounds.

• Driving may be necessary within company-designated geographical areas.
