Government Programs Care Manager, Support Specialist II

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Engage with members and community partners to enhance healthcare initiatives and activities.

• Assist in educational and quality improvement campaigns.

• Help in scheduling appointments for care managers and members.

• Carry out ongoing tasks related to members' health-related social needs, either directly or through the social service department and community organizations.

• Address inquiries from members.

• Aid clinicians in managing physical and behavioral health care for members, including provider and member activities.

• Foster relationships with community partners.

• Collaborate with community-based organizations.

• Contribute to the development of community health outreach programs.

• Conduct fieldwork as necessary for initiatives, health campaigns, and departmental operations.

• Oversee low-risk members regarding health-related social needs, education, and basic outreach screenings, assessments, and inquiries.

⛳️ Requirements

• Bachelor’s Degree OR 1 year of college plus 3 years of experience in health, social services, or community work, OR 4 years of experience in health, social services, or community settings.

• Familiarity with medical terminology.

• Background in coordinating member-related needs, assisting members, and analyzing member requirements.

• Proficient in PC applications, including Microsoft Office.

• Strong customer service abilities.

• Excellent verbal and written communication skills, including the ability to draft written correspondence for members and other departmental staff.

• Coaching skills, including motivational interviewing techniques, to educate members on medical and behavioral health issues.

• Sponsorship is not available.
