Field Case Manager

This is a fully remote position, open to applicants in New Jersey, +1 more state.

📋 Description

• Act as an advocate for injured workers, their families, and caregivers throughout the rehabilitation journey.

• Provide support, guidance, and coordination of care for injured workers.

• Help injured workers attain recovery and independence through advocacy, effective communication, education, resource identification, and service facilitation.

• Engage directly with injured workers and healthcare professionals.

• Promote progressive medical treatment and ensure timely return-to-work transitions.

• Serve as a liaison, facilitating communication among all stakeholders involved.

• Identify suitable providers and facilities throughout the service continuum.

• Ensure resources are utilized efficiently and cost-effectively.

• Operate from a home-based office while frequently traveling within the designated area.

⛳️ Requirements

• A bachelor’s degree in a health or human services discipline is mandatory.

• One of the following certifications is required: CCM, CDMS, or CRC.

• RN licensure is preferred.

• A valid driver's license is essential.

• High-speed internet is necessary.

• Capability to engage face to face with injured workers and healthcare providers.

• Knowledge and experience in medical/clinical or rehabilitation fields.

• Strong analytical and conceptual thinking skills.

• Exceptional judgment, troubleshooting, problem-solving, and discretion abilities.

• Capacity to manage work-related stress effectively.

• Ability to prioritize and handle multiple tasks simultaneously.

• Competence in meeting deadlines.

• Skill in using a computer keyboard.

• Willingness to travel as needed.

• Employment may be subject to the successful completion of a job-related MVR check.
