Field Care Manager Nurse 2 – BSN

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Evaluate and assess the needs and requirements of members to promote or sustain optimal wellness.

• Direct members and their families towards suitable care and wellbeing resources.

• Facilitate connections with community, care, and social service resources.

• Address members' physical, environmental, and psychosocial health challenges using various strategies and techniques.

• Identify and eliminate obstacles to effective care.

• Continuously oversee care through assessments and evaluations.

• Ensure that members are making progress towards their desired outcomes.

• Develop care plans for members when necessary.

• Collaborate with staff, providers, members, and families through in-person meetings.

• Travel between 75%-90% of the time to facilitate field collaboration and member engagement.

⛳️ Requirements

• Bachelor's degree in nursing (BSN).

• Current nursing license in South Carolina without any disciplinary actions.

• Must be a resident of the State of South Carolina.

• A minimum of 2 years of experience in case or care management.

• At least 2 years of experience working with the behavioral health population.

• Familiarity with community health and social service agencies as well as community resources.

• Proficiency in using electronic information applications and software programs, including electronic medical records.

• Valid driver's license from the state.

• Personal vehicle liability insurance that meets state minimum requirements or Humana's specified limits, whichever is higher.

• Availability to work Monday through Friday, from 8:00 AM to 5:00 PM Eastern Time.

• Capability to meet home internet requirements of a minimum of 25 Mbps download and 10 Mbps upload speeds.

• Ability to work from a designated space without interruptions to safeguard PHI/HIPAA information.
