Family Nurse Practitioner – Homevisits, Telehealth

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Conduct annual risk assessments in residential environments or through telehealth, adhering to the nurse practitioner scope of practice.

• Execute thorough in-home health risk evaluations to identify both active and chronic health conditions, as well as physical, mental, and social requirements.

• Gather patient histories, perform assessments, and establish diagnoses.

• Provide written records of patient visits in compliance with NCQA standards.

• Measure patient vital signs as needed.

• Initiate case management referrals and liaise with primary care providers as required.

• Communicate effectively with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnoses.

• Carry out clinical responsibilities while adhering to OSHA Universal Precautions.

• Uphold patient confidentiality at all times.

• Attend mandatory meetings and in-service training, and engage in committees when requested.

• Take part in professional development opportunities and maintain necessary professional licenses and affiliations.

⛳️ Requirements

• Valid and unrestricted nurse practitioner license to operate in designated coverage states.

• Board certification through ANCC, AANP, or NCCPA.

• Current Basic Life Support (BLS) certification.

• Active participation in Medicare/Medicaid programs.

• Possession of a valid driver’s license with a clean driving record.

• Exceptional electronic health record (EHR) capabilities.

• Geriatric training or experience is preferred.

• Capacity to adjust to varying weather conditions and diverse home/residential settings of patients.

• Ability to perform a full range of body movements, including the handling and lifting of patients.

• Regularly lift and carry items weighing up to 50 pounds.

• Capability to travel in an automobile or van for up to 150 miles daily in both urban and rural environments.

• Willingness to drive when necessary.
