This is a fully remote position, open to applicants in Tennessee.
• Provide support to enhance members’ physical, behavioral, and social health and well-being.
• Assist members in securing and sustaining competitive, integrated employment opportunities.
• Navigate complex relationships to ensure members have access to the essential services they require.
• Conduct intake, enrollment, and continuous assessments of members.
• Create and uphold Person-Centered Plans.
• Oversee and ensure the quality of overall service delivery.
• Collaborate with members and their families to identify necessary supports and services that aid in health, employment, and independent or community living.
• Work alongside clinical and social support teams to address members’ needs in physical and behavioral health, as well as long-term services.
• Execute comprehensive, objective face-to-face assessments to evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Utilize established criteria to approve suitable home- and community-based services, ensuring that services are delivered and members’ needs are satisfactorily met.
• Travel throughout Sumner County to visit members at their residences.
• Registered Nurse (RN); or a Bachelor’s degree in social work, nursing, education, human services, healthcare profession, or a related field; or compliance with federal QDDP/QIDP standards.
• Alternatively, if lacking a degree: a minimum of 5 years experience as an independent Support Coordinator or case manager for service recipients under the 1915(c) HCBS Waiver, along with completion of Personal Outcome Measures Introduction and Assessment Workshop trainings.
• A minimum of 2 years of experience in the healthcare sector or a similar operations environment is required.
• Current CDDN certification is preferred for RNs, or DDC certification for LPNs, as applicable.
• Licensed Master Social Worker or Licensed Clinical Social Worker credentials are preferred.
• Must possess a valid driver's license.
• Various immunizations and/or related medical tests may be necessary.
• A digital literacy assessment is mandatory.
• Proficiency in Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint) is required.
• Capability to work both independently and collaboratively within a team setting.
• Strong problem-solving skills and the ability to handle multiple tasks with tight deadlines.
• Demonstrated organizational skills, reliability, strong work ethic, and the ability to manage several responsibilities simultaneously.
• Exceptional oral and written communication abilities.
• Strong interpersonal and organizational competencies.
• Drivers operating vehicles for company-related duties must adhere to the BCBST Driver Safety Program, which includes having a valid license, auto insurance compliance, and an acceptable motor vehicle record.
• Ideal candidates should reside in Sumner County or within a 35-mile radius.
• Mileage reimbursement for travel to members’ residences.
• A work schedule from 8:00 am to 5:00 pm with no on-call obligations.
