This is a fully remote position, open to applicants in Tennessee.
• Assist members in enhancing their physical, behavioral, and social health and overall well-being.
• Support members in securing and sustaining competitive, integrated employment opportunities.
• Navigate complex relationships to guarantee that members receive essential services.
• Conduct intake, enrollment, and continuous assessments.
• Create and uphold Person-Centered Plans.
• Oversee and maintain the delivery of services.
• Collaborate with members and their families to determine required supports and direct services that align with personal objectives.
• Work jointly with clinical and social support teams to address members’ physical, behavioral health, and long-term service requirements.
• Perform comprehensive, unbiased face-to-face visits, evaluating members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Use established criteria to authorize suitable home- and community-based services, ensuring that services are provided and members’ needs are fulfilled.
• Travel to members’ residences throughout the Cumberland, Roane, Morgan, and Blount County regions.
• RN designation, with a preference for current Developmental Disabilities Nurses Association certification as a Certified Developmental Disabilities Nurse (CDDN) for RNs or Developmental Disabilities Nurse (DDC) for LPNs.
• Alternatively, a B.S. in social work, nursing, education, human services, health care profession, or a related discipline.
• Preference for a Licensed Master Social Worker or Licensed Clinical Social Worker.
• Or meet federal qualifications for a Qualified Developmental Disabilities Professional (QDDP or QIDP).
• Alternatively, 5+ years of experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, along with completed Personal Outcome Measures Introduction and Assessment Workshop trainings.
• Minimum of 2 years of experience in the healthcare sector or a similar operational environment is required.
• Must possess a valid driver's license.
• Various immunizations and/or associated medical evaluations may be necessary.
• Digital literacy assessment is required.
• Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
• Capability to work independently with minimal supervision or collaboratively within a team.
• Strong problem-solving skills and the ability to manage multiple tasks with critical deadlines.
• Excellent oral and written communication abilities.
• Strong interpersonal and organizational skills.
• Must demonstrate organization, reliability, and the capacity to handle various tasks simultaneously.
• For regular vehicle use: valid driver’s license, compliant auto insurance, and acceptable motor vehicle record.
• Availability for a work schedule from 8:00 AM to 5:00 PM.
• Mileage reimbursement for travel to members’ residences.
• Full-time employment position.
• No on-call duties required.
