ECF Choices Support Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Assist members in achieving their physical, behavioral, and social health and well-being.

• Help members secure and sustain competitive, integrated employment opportunities.

• Manage complex relationships to guarantee that members access essential services.

• Conduct intake, enrollment, and continuous assessments.

• Create and uphold Person-Centered Plans.

• Oversee and ensure the effective provision of services.

• Collaborate with members and their families to identify supports and direct services aimed at health, employment, and goals for independent or community living.

• Work in partnership with clinical and social support colleagues to address members’ physical, behavioral health, and long-term service requirements.

• Perform detailed, objective face-to-face visits and evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.

• Utilize criteria to approve suitable home- and community-based services.

• Verify that authorized services are delivered and that members' needs are fulfilled.

• Operate on a schedule from 8:00 a.m. to 5:00 p.m. without on-call responsibilities.

• Travel to members’ residences in the Greene, Hamblen, and Jefferson County region.

⛳️ Requirements

• Registered Nurse (RN) with a preference for current certification from the Developmental Disabilities Nurses Association as a Certified Developmental Disabilities Nurse (CDDN) for RNs or Developmental Disabilities Nurse (DDC) for LPNs, as applicable.

• Alternatively, a Bachelor of Science in social work, nursing, education, human services, health care profession, or another related field.

• Or, fulfill the federal criteria for a Qualified Developmental (or Intellectual) Disabilities Professional (QDDP or QIDP).

• Alternatively, in place of a degree, possess over 5 years of experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, along with completion of the Personal Outcome Measures Introduction and Assessment Workshop trainings established by the Council on Quality and Leadership.

• Licensed Master Social Worker or Licensed Clinical Social Worker is preferred.

• A minimum of 2 years of experience in the healthcare industry or an operations-type setting is required.

• A valid driver's license is necessary.

• Various immunizations and/or relevant medical tests may be required.

• Capability to work independently with minimal supervision or collaboratively in a team environment.

• Proficiency in Microsoft Office applications (Outlook, Word, Excel, and PowerPoint).

• Skill in problem-solving and managing multiple assignments under critical deadlines.

• Well-organized, dependable, and capable of handling various tasks.

• Exceptional oral and written communication abilities.

• Strong interpersonal and organizational skills are essential.

• A digital literacy assessment will be required.

• Employees who regularly operate a company, personal, or rental vehicle must uphold a valid driver's license, adhere to auto insurance requirements, and maintain an acceptable motor vehicle record.
