This is a fully remote position, open to applicants in United States.
• Oversee the workforce of community health workers and Community Supports through two leaders across twelve counties and eight health plan contracts.
• Take ownership of the operational performance of the community health worker workforce and Community Supports.
• Establish the geographic scope and hiring strategies by county, aligning with care model conversion schedules and enrollment projections.
• Merge community teams and clinical care teams into a unified care team.
• Maintain accountability for audits related to community-based operations, ensuring compliance with health plan fidelity requirements and DHCS readiness.
• Present operational performance metrics in health plan forums.
• Collaborate with Finance to manage unit economics for Community Supports and the CHW Benefit, including recommendations on the continuation of the CHW Benefit.
• Create career pathways, training frameworks, and management structures for an expanding frontline workforce.
• Implement first-year deployment strategies, integrated workflows, management structures, contribution measures, and performance metrics that link CHW engagement to recognized outcomes.
• A minimum of eight years of experience leading community health, care management, or population health programs, including managing through other leaders.
• Proven experience managing distributed frontline teams across multiple counties or markets.
• Familiarity with Medicaid programs.
• CalAIM experience is highly preferred.
• Significant advantage for those with direct experience in Enhanced Care Management or Community Supports.
• Demonstrated success in integrating community-based teams into clinical care delivery rather than operating them separately.
• Proficiency in using operational data to inform decisions and the ability to justify those decisions to a health plan.
• Experience with audit and regulatory readiness in a Medicaid context.
• Capability to design operational workflows and management frameworks for a growing frontline workforce.
• Background in a tech-enabled delivery organization, especially during periods of operational transformation.
• Expertise in crafting career pathways and training frameworks for frontline personnel.
• Experience with housing navigation or other social services programs offered under a Medicaid benefit.
• Bilingual in Spanish or experience leading teams that primarily serve Spanish-speaking members.
• An advanced degree in public health, social work, healthcare administration, or a related field is preferred.
