Complex Discharge Planner

This is a fully remote position, open to applicants in Arizona, +7 more states.

📋 Description

• Develop, evaluate, and coordinate intricate discharge plans for high-risk members based on comprehensive needs assessments.

• Ensure seamless transitions from acute care to subsequent levels of care.

• Educate members along with their families/caregivers regarding discharge plans.

• Collaborate with providers, community organizations, and utilization review personnel to create safe, effective, and clinically appropriate transition plans.

• Oversee discharge plans across various levels of care and facilities.

• Collect, document, and preserve discharge plans within member medical records and health management systems.

• Provide guidance on discharge plans and utilization procedures.

• Offer leadership insights on enhancements to the discharge planning process.

• Facilitate and assist with prior authorization processes related to discharge planning.

• Support the concurrent review team as necessary.

• Carry out additional responsibilities as assigned.

• Adhere to all established policies and standards.

⛳️ Requirements

• Graduate from an accredited nursing program or hold a Bachelor’s degree in Nursing.

• 2–4 years of relevant experience.

• Minimum of 2 years of experience in acute care is required.

• Active Compact RN License.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure.

• Preferred clinical knowledge of discharge planning competencies.

• Familiarity with Medicare and Medicaid regulations is preferred.

• Understanding of utilization management processes is preferred.
