Complex Case Manager III

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Deliver member-focused, evidence-driven case management across various products

• Perform thorough assessments of members

• Create, execute, and oversee personalized care plans

• Coordinate services throughout the healthcare continuum

• Collaborate with members, families, caregivers, providers, and interdisciplinary teams

• Assist in alleviating the burden of illness for individuals and families while lowering healthcare expenses

• Guide members with the greatest needs through the health system

• Undertake additional responsibilities as assigned

⛳️ Requirements

• Associate's degree in Nursing or relevant work experience in a related area

• Minimum of 3 years of experience beyond the degree in clinical or acute care nursing

• Current, unrestricted RN license in the state of residence

• Certified Case Manager (CCM) designation preferred, or willingness and commitment to obtain within two years of employment

• Experience with vulnerable and complex populations, including diverse age groups and various ethnic and socioeconomic backgrounds

• Background in clinical, home care, or telephonic settings

• Direct case management experience is highly preferred

• Familiarity with managed care principles and regulatory guidelines is preferred

• Proficient in electronic health records and care management software

• Strong verbal and written communication skills, with the ability to present in group settings

• Capable of working efficiently to meet departmental benchmarks

• Ability to make decisions under pressure and manage multiple complex issues on a daily basis

• Must have legal authorization to work in the United States at the time of application
