Complex Care Registered Nurse, California RN License Required, Bilingual English/Spanish Preferred

This is a fully remote position, open to applicants in California.

📋 Description

• Take charge of the member journey and oversee care coordination for high-acuity Medicare Advantage members.

• Manage the transitions of care for members who are being discharged from various inpatient facilities.

• Perform medication reconciliations and carry out clinical monitoring.

• Act as a clinical escalation point within the pod.

⛳️ Requirements

• At least 3 years of clinical RN experience.

• Experience in managing medically complex and high-risk patient populations.

• Previous experience in a Medicare Advantage or managed care setting.

• Familiarity with telehealth or virtual care delivery models.

• An Associate Degree in Nursing (ADN) is required; a BSN is preferred.

• An active, unrestricted Registered Nurse (RN) license in the relevant state(s).

• Current BLS certification.

• Preferred familiarity with HEDIS, HCC coding, and care gap management.

• Proficient in using telehealth delivery platforms.
