This is a fully remote position, open to applicants in United States.
• Conduct detailed and focused evaluations of patients with hypertension through virtual consultations, which include ordering necessary diagnostics, interpreting laboratory and imaging results, and collaborating with the specialty care clinic team to develop a treatment plan.
• Create precise diagnoses and devise personalized treatment plans for patients, encompassing medication management, lifestyle changes, and monitoring advice.
• Reduce the risk of cardiovascular disease by proactively initiating and managing statin therapy for individuals with diabetes and/or cardiovascular conditions.
• Collaborate with a multidisciplinary care team—including primary care providers, specialists, and dedicated team members such as pharmacy, social work, and health coaches—as well as family members and caregivers to ensure holistic, patient-centered care, maintain continuity, and implement a collaborative care strategy.
• Act as the clinical advisor and provide clinical escalation support for the specialty clinic staff and other teams during business hours.
• Engage in regular panel review discussions to offer insights and guidance regarding medical management.
• Utilize our proprietary electronic health information system for patient visits while also providing feedback on how to enhance the interface.
• Keep accurate and current patient medical records, ensuring adherence to relevant legal and ethical standards.
• Get involved in quality improvement initiatives and continuous professional development to remain updated on best practices and advancements in hypertension and atherosclerotic cardiovascular disease risk reduction.
• Comply with all pertinent laws, regulations, and industry standards, including those related to patient privacy and telehealth practices.
• A valid and clear RN and APRN or Physician Assistant license must be maintained during employment at Devoted Medical, with a commitment to obtaining and maintaining multiple state licenses.
• At least 5 years of experience in outpatient clinical practice, including a minimum of 2 years focused in primary care or a relevant subspecialty (e.g., cardiology, nephrology, endocrinology) with a strong emphasis on managing hypertension and hyperlipidemia.
• A robust clinical background in both primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD), including understanding disease processes, evidence-based treatment approaches, medication management, and lifestyle modifications.
• Availability to work Monday-Friday from 8:30am-5:00pm MST or PST preferred; EST and CST applicants will also be considered.
• Fully bilingual in Spanish and English.
