Bilingual RN Care Manager

This is a fully remote position, open to applicants in Florida.

📋 Description

• Identify and address obstacles that impede effective patient care.

• Ensure that patients are making progress towards their desired outcomes by overseeing their care through assessments and evaluations.

• Track member progress and modify care strategies to guarantee optimal results.

• Aid in the creation and execution of personalized care plans.

• Collaborate with interdisciplinary teams to synchronize care and services.

• Manage daily responsibilities, maintain precise documentation, and adhere to established guidelines and procedures.

• Utilize your skills to make a significant impact.

⛳️ Requirements

• Bachelor's Degree in Nursing (BSN).

• Bilingual in English and Spanish, with the capability to pass language proficiency assessments in both languages.

• Valid RN license in Puerto Rico without restrictions.

• Valid RN license in Florida without restrictions.

• Membership with the CPEPR (Colegio de Profesionales de Enfermería de Puerto Rico).

• Previous clinical experience in adult acute care, skilled nursing, rehabilitation, or discharge planning.

• Knowledge of chronic condition management (including treatment, pharmacological treatment, signs, and symptoms) for conditions such as Diabetes, Hypertension, COPD, and chronic kidney disease.

• Availability to work an 8-hour shift between 8:30 AM - 5:30 PM EST, adjusted for Daylight Savings.
