Community Resource Coordinator II

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Facilitate connections for members to community and care resources in a cost-efficient manner

• Organize community outreach initiatives and enhance awareness of available care/services

• Assist members with inquiries regarding community and care resources

• Create correspondence and educational materials for members

• Document and uphold community resources in compliance with health plan standards

• Support nurses and social workers through outreach, community education, informal guidance, and member assistance

• Perform non-clinical general health assessments and refer members to suitable care/services

• Execute non-medical assessments focusing on home safety, community/environment resources, transportation, employment, and additional needs

• Conduct telephonic and/or in-person outreach to engage hard-to-reach individuals and families

• Visit individual residences and/or community organizations as required

• Carry out other assigned duties

⛳️ Requirements

• High School diploma or GED

• 1–2 years of relevant experience

• Familiarity with Social Determinants of Health (SDOH) barriers

• Capability to work remotely from home anywhere within the state of Missouri

• Proficiency in providing telephonic support to foster care members

• Availability Monday–Friday, 8am–5pm

• Ability to conduct telephonic and/or in-person outreach

• Willingness to make visits to individual homes and/or community organizations as necessary
