Community Health Worker – Field

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Act as an intermediary between Aetna members and the healthcare system through outreach and educational initiatives.

• Perform home visits for members dealing with chronic conditions.

• Reinforce care plans while adapting tasks to align with members’ cultural, linguistic, and religious backgrounds.

• Inform healthcare professionals and providers about social determinants that impact health.

• Cultivate and sustain relationships with members, providers, nurse case managers, agencies, supervisors, and office personnel.

• Mitigate cultural and socioeconomic obstacles between clients and institutions.

• Aid in the execution of care plans and provide member education during home visits.

• Formulate care management strategies and connect members to health and social services.

• Assist members in recognizing socioeconomic challenges and creating health/social management plans and objectives.

• Identify deficiencies in community care systems and support members in accessing community services, scheduling appointments, securing transportation, and submitting program applications.

• Promote medication adherence by reviewing medication lists and facilitating refills.

• Instruct members on disease self-management, covering topics like nutrition and symptom monitoring.

• Accompany members to appointments as necessary.

• Maintain member records in electronic health record systems and finalize reports and program documentation.

• Travel extensively to client residences, community venues, agencies, and outreach locations.

⛳️ Requirements

• Must live in one of the following parishes: Orleans, St. Bernard, Jefferson, Plaquemines.

• Willingness to travel 70% of the time.

• Access to reliable transportation is essential.

• 3-5 years of relevant work experience is required.

• 1-2 years of experience in providing clinical support services is necessary.

• A minimum of 1 year of community outreach experience and familiarity with Louisiana community resources.

• At least 1 year of experience working with clients who have clinical needs (physical and/or behavioral health).

• Proven proficiency in using a personal computer, keyboard navigation, and MS Office Suite applications, including MS Teams, Outlook, Word, Excel, and Electronic Health Record Systems.

• Flexibility to work outside of core business hours, Monday to Friday, from 8 am to 5 pm.

• Capability to work 2 days weekly until 9 pm local time.

• Community Health Worker Certification or equivalent training and experience is preferred.

• Bilingual proficiency in Spanish or French is an advantage.
