Community Health Worker – Certified

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Collaborate with the community to provide outreach to patients who have complex medical and social needs.

• Serve as a resource to assist patients in navigating the healthcare system.

• Train and evaluate new Community Health Workers.

• Continuously assess staff proficiency during orientation and offer support for training advancement or remediation.

• Identify and reach out to patients with medical complexities.

• Work alongside multidisciplinary teams to create and execute care plans.

• Engage with patients through phone calls, home visits, hospital visits, and by accompanying them to healthcare appointments when necessary.

• Measure vital signs.

• Detect gaps in care and connect patients with appropriate resources.

• Facilitate telemedicine appointments between members and providers.

• Provide education specific to various diseases.

• Conduct medication reviews to ensure adherence to prescribed treatments.

• Perform safety evaluations within patients' homes.

• Support adherence to treatment plans by identifying and addressing care gaps early on.

• Communicate any concerns to the clinical team and management, seeking support and guidance proactively.

• Participate in multidisciplinary team meetings to enhance treatment plans.

• Reinforce treatment plans as directed by the Case Manager and Licensed Social Worker.

• Offer consultation and case reviews as necessary.

⛳️ Requirements

• High School Diploma or Equivalent (GED) is required.

• A minimum of 1 year of relevant work experience is required.

• Certification as a Community Health Worker (CCHW) from the Pennsylvania Certification Board is necessary.

• Proficient clerical skills.

• Strong communication skills.

• Ability to multitask effectively.

• Proven problem-solving abilities.

• Availability to work daytime hours.

• Capability to commit to 40 scheduled hours per week.
