Community Care Coordinator

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver proactive support to patients, including customer service, advocacy, emotional assistance, and educational resources for individuals and families dealing with limb loss and orthotic/prosthetic requirements.

• Ensure smooth care delivery for patients with amputations or limb differences under the guidance of the ACM/CM or their designated representative.

• Offer consultation, comfort, and direction from the initial referral through surgery, recovery, treatment, rehabilitation, and the final device delivery.

• Reach out to new patients before or after their initial consultation or AmpuShield fitting and input their information into Hanger Clinic’s patient management and tracking system.

• Assist patients and families in the weeks leading up to and following surgery by providing information, resources, advice, and counseling.

• Organize AMPOWER visits.

• Develop and sustain referral relationships for Hanger Clinic.

• Aid in obtaining clinical documentation necessary for insurance reimbursement.

• Maintain patient engagement through follow-up communications and scheduled appointments.

• Provide updates on surgeon follow-up appointments, including wound descriptions, photographs, care plans, and necessary modifications.

• Conduct visits to hospitals, skilled nursing facilities, and rehabilitation centers as determined by the care team.

• Identify and follow up on referrals to ensure consistent care and address patient/family needs.

• Act as a point of contact for patients, families, physicians, external agencies, staff, and the general public regarding clinic services.

• Serve as a liaison between patients and clinic areas or ancillary services to coordinate pre-operative and post-operative planning, preparation, and care.

• Utilize clinical communication and tracking tools to maintain ongoing communication with patients and healthcare professionals.

• Assist patients with upcoming visits and fittings, which may require ABC Certified Orthotic Fitter credentialing.

• Collaborate with Hanger personnel and office staff to manage daily communication with patients, care team members, and referral partners.

• Engage with community healthcare providers to build relationships and create opportunities to serve additional patients.


⛳️ Requirements

• At least 2 years of experience in a patient care setting.

• A BA/BS in a health-related field or equivalent professional experience.

• Capability to be credentialed in major hospital systems.

• Proven experience delivering high-level customer service in a fast-paced environment.

• Experience in managing confidential data with discretion.

• Must possess, or be eligible to obtain, a valid driver’s license and maintain a driving record in accordance with Hanger’s Motor Vehicle Safety Policy and Procedures.

• Excellent interpersonal, oral, and written communication abilities.

• Strong customer service skills.

• An empathetic, kind, and engaging demeanor.

• Non-revoked, valid passenger vehicle driver’s license.

• Outstanding organizational skills.

• Knowledge of insurance processes.

• A clinical background is beneficial and can be supplemented through training.

• Time management skills and the ability to adhere to a schedule.

• Preferred certifications include ATC, EMT, MA, Surgical Tech, LVN, PTA, Physician Extenders, OTA, Social Work, or equivalent experience.

• Experience with progressive responsibilities in healthcare facility administrative support roles, such as insurance processing coordinator, patient care coordinator, MA, or similar positions.

• Experience interacting with physicians, nurses, physical therapists, occupational therapists, and clinical support staff.

• Willingness to work with individuals with disabilities.

• Proficiency in SharePoint and Post-Acute CRM.

• Understanding of medical reimbursement and terminology.

• Familiarity with Hanger Clinic front-office duties is preferred.

• Competency in using computers, Windows-based software, email, and essential office equipment.

• Commitment to acting with integrity, honesty, transparency, and respect.
