Clinical Triage Nurse, RN – Wed-Sat

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Manage incoming sick calls from patients within the Medical Group

• Evaluate patient symptoms and deliver suitable medical guidance

• Assess the necessary level of care and coordinate with the care team

• Analyze clinical test results and summarize findings to support providers

• Prioritize and address incoming phone calls, voicemails, and test results

• Conduct comprehensive telephone assessments utilizing probing questions and the EMR

• Utilize evidence-based guidelines to determine triage priorities

• Recognize crises and link patients to emergency services following protocol

• Provide education to patients and caregivers regarding health maintenance, disease prevention, self-care, medications, and follow-ups

• Make outbound calls to relay provider orders

• Work collaboratively with care teams to ensure patient needs and follow-ups are met

• Evaluate test result data and convey findings to providers

• Enter data for patient results and quality metrics

• Address and escalate patient complaints, grievances, and concerns

• Engage in projects and initiatives aimed at quality improvement

• Undertake additional responsibilities assigned by the Clinical Triage Manager and/or Director

⛳️ Requirements

• Graduate from an accredited nursing education program as a Registered Nurse

• Current and valid RN license

• Must obtain and maintain RN license across all service states within six months of hiring

• Successful completion of necessary nursing continuing education requirements

• Ability to respond decisively and promptly in urgent or emergency situations

• Strong understanding of medical terminology and pharmaceuticals

• Proven ability to handle multiple tasks simultaneously

• Excellent communication skills (both written and verbal)

• Demonstrated proficiency in utilizing electronic health records (EHR) systems

• Effective communication with providers, patient care teams, patients, family members, and facility staff

• Ability to work independently and remotely

• Knowledge of fundamental principles, practices, and techniques in primary healthcare

• Strong organizational abilities and attention to detail

• Conflict resolution capabilities

• Familiarity with Medicare and Medicaid insurances and programs

• Over 2 years of experience as a Registered Nurse

• Experience in a contact center environment

• Experience in nurse triage

• Familiarity with Aprima (EHR)

• Multistate License (MSL)
