Clinical Triage Nurse, RN – Wed-Sat

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Manage incoming sick calls from patients of the Medical Group.

• Evaluate patient symptoms and deliver suitable medical guidance.

• Ascertain the level of care needed and collaborate with the care team for coordination.

• Review clinical test outcomes and prepare summaries for provider evaluation.

• Prioritize and address incoming phone calls, voicemails, and test results.

• Conduct comprehensive telephone assessments utilizing probing questions and the EMR.

• Implement evidence-based guidelines to establish triage priorities.

• Recognize crisis situations and connect patients with emergency services following established protocols.

• Educate patients and caregivers on health maintenance, disease prevention, self-care, medications, and necessary follow-up actions.

• Place outbound calls to relay provider orders.

• Work in conjunction with care teams to ensure follow-ups and enhance quality outcomes.

• Analyze data from results and share insights with providers.

• Input patient results and quality metrics accurately.

• Address and escalate any patient complaints, grievances, and concerns as needed.

• Engage in projects and quality enhancement initiatives.

• Undertake additional responsibilities as assigned by the Clinical Triage Manager and/or Director.

⛳️ Requirements

• Graduate from an approved or accredited nursing education program as a Registered Nurse.

• Current RN license that is valid.

• Must be able to acquire and sustain RN licensure in all service states within 6 months of hire.

• Successful completion of all required nursing continuing education obligations.

• Ability to respond decisively and swiftly in urgent and emergent scenarios.

• Strong understanding of medical terminology and pharmaceuticals.

• Proven experience in managing multiple tasks simultaneously.

• Excellent communication skills, both written and verbal.

• Demonstrated proficiency in utilizing electronic health records (EHR) systems.

• Ability to effectively communicate with providers, patient care teams, patients, their family members, and facility staff.

• Capacity to work independently and remotely.

• Familiarity with fundamental principles, practices, and techniques in primary healthcare.

• Strong organizational abilities and meticulous attention to detail.

• Skills in conflict resolution.

• Knowledge of Medicare and Medicaid insurances and programs.

• Over 2 years of experience as a Registered Nurse.

• Experience in a contact center environment.

• Background in nurse triage.

• Experience with Aprima (EHR) system.

• Possession of a Multistate License (MSL).
