This is a fully remote position, open to applicants in Michigan.
• Manage incoming sick calls from patients of the Medical Group.
• Evaluate patient symptoms and offer appropriate medical guidance.
• Assess the necessary level of care and coordinate with the care team.
• Analyze clinical test results and summarize findings to support providers.
• Prioritize and handle incoming phone calls, voicemails, and test results.
• Evaluate patients’ symptoms and medical history via phone using probing inquiries and the EMR.
• Utilize evidence-based guidelines to assess urgency and assign triage priority.
• Recognize crisis situations and connect patients with emergency services as per protocol.
• Educate patients, caregivers, and others on health maintenance, disease prevention, self-care, medications, and follow-up procedures.
• Conduct outbound calls to relay provider orders.
• Collaborate with care teams to complete patient follow-ups and enhance quality outcomes.
• Communicate with patients and families, addressing their concerns and providing emotional support.
• Analyze test-result data and relay needs, patterns, and variances to providers.
• Enter patient results and quality metrics following established protocols.
• Maintain empathetic written and verbal communication with care team members.
• Comply with company policies, quality standards, and productivity expectations.
• Identify, acknowledge, escalate, and address patient complaints, grievances, and concerns.
• Engage in assigned projects and initiatives aimed at quality improvement.
• Carry out additional tasks assigned by the Clinical Triage Manager and/or Director.
• Completion of an approved or accredited nursing education program as a Registered Nurse.
• A valid RN license that is current.
• Must secure and maintain RN licensure in all service states within 6 months of hire.
• Successful completion of required continuing education for nursing.
• Ability to act decisively and swiftly in urgent and emergency situations.
• Strong understanding of medical terminology and pharmaceuticals.
• Proven experience in managing multiple tasks simultaneously.
• Excellent communication skills, both written and verbal.
• Demonstrated proficiency in using electronic health records (EHR) systems.
• Effective communication skills with providers, patient care teams, patients, their family members, and facility staff.
• Capability to work remotely and independently.
• Knowledge of fundamental principles, practices, and techniques in primary healthcare.
• Strong organizational abilities and meticulous attention to detail.
• Skills in conflict resolution.
• Familiarity with Medicare and Medicaid insurances and programs is preferred.
• 2+ years of experience as an RN is preferred.
• Experience in a contact center setting is preferred.
• Nurse triage experience is preferred.
• Experience with Aprima (EHR) is preferred.
• Possession of a Multistate License (MSL) is preferred.
• Candidates may be required to undergo a background check in the state of Florida.
