Clinical Social Worker – LCSW

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of high-risk patients with social complexities

• Perform in-person and telephonic psychosocial evaluations with patients and/or their caregivers

• Explore psychological and social determinant challenges and create suitable interventions to address care gaps and needs

• Offer short-term counseling services as necessary

• Coordinate services to ensure that patients and families comprehend treatment plans

• Collaborate with the care team to attain quality and patient experience outcomes, including participation in High-Risk Huddles

• Engage with community-based services to enhance patients’ capacity to age in place

• Establish and execute shared care goals with patients and their families

• Partner with community agencies to build relationships and provide resources that foster patient independence

⛳️ Requirements

• Master’s degree in social work

• Must hold a valid LCSW or LISW license (as per state regulations)

• Must possess a current driver’s license and maintain a clean driving record

• Ability to exercise sound clinical judgment and communicate effectively in both written and spoken forms

• Proficient computer skills

• Capacity to work independently and communicate efficiently with professional staff across various disciplines and programs

• Ability to effectively plan time, juggle multiple tasks, work within tight deadlines, resolve issues, identify patient service trends, determine system enhancements, and implement changes

• Capability to share knowledge with others and recognize the importance of promoting performance improvement while ensuring patient satisfaction and efficiency

• Preferred: Experience in Care Management within a community-based environment or similar

• Preferred: Experience with chronic, complex, and/or behavioral health populations
