Clinical Social Worker, LCSW

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of patients facing high-risk and socially intricate challenges.

• Perform both in-person and telephonic psychosocial evaluations with patients and/or their caregivers.

• Examine psychological and social determinants that hinder care and devise interventions to bridge those gaps.

• Offer short-term counseling services when necessary.

• Coordinate services to ensure that patients and families comprehend their treatment plans.

• Collaborate with the care team to meet quality and patient experience objectives, including participation in High-Risk Huddles.

• Connect with community-based services to enhance patients’ ability to age in place.

• Establish and execute shared care goals with patients and their families.

• Partner with community agencies to build relationships and provide resources that promote patient independence.

⛳️ Requirements

• Master’s degree in social work.

• Must hold a valid LCSW or LISW license (as required by state regulations).

• Must possess a valid driver’s license and maintain a clean driving record.

• Ability to apply sound clinical judgment and communicate effectively in both written and verbal forms.

• Proficient computer skills.

• Capable of being self-directed and effectively communicating with professional staff across various disciplines and programs.

• Skilled in time management, able to balance multiple tasks, work under tight deadlines, resolve issues, identify patient service trends, recommend system improvements, and implement changes.

• Ability to share knowledge with others and recognize the importance of fostering performance improvement while ensuring patient satisfaction and efficiency.

• Preferred experience in Care Management within a community-based setting or equivalent.

• Preferred experience working with chronic, complex, and/or behavioral health populations.

• Candidates may be required to undergo a background check in the state of Florida.
