Clinical Social Worker

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of patients who are socially complex and at high risk.

• Perform psychosocial assessments through both face-to-face interactions and telephonic communication with patients and/or their caregivers.

• Examine psychological and social determinant obstacles and create interventions to bridge gaps in care and address needs.

• Offer short-term counseling services as required.

• Facilitate coordination of services to ensure that patients and families comprehend the treatment plans.

• Collaborate with the care team to achieve quality outcomes and enhance patient experience, including active involvement in High-Risk Huddles.

• Conduct outreach to community services that assist patients in aging in place.

• Collaborate with patients and families to create and implement shared care goals.

• Partner with community-based organizations to foster relationships and provide resources that promote patient independence.

⛳️ Requirements

• A Master’s degree in social work is required.

• Must hold a valid license as an LCSW or LISW (depending on the state).

• A valid driver's license and a clean driving record are mandatory.

• Ability to apply sound clinical judgment and communicate effectively in both written and verbal forms.

• Proficient computer skills are essential.

• Must be self-motivated and capable of communicating effectively with professional staff across various disciplines and programs.

• Ability to effectively plan time, manage multiple tasks, work within strict deadlines, resolve issues, identify patient service trends, determine system enhancements, and implement changes.

• Capability to share knowledge with others while understanding the importance of fostering performance improvement alongside patient satisfaction and efficiency.

• Experience in Care Management within a community-based setting or similar (preferred).

• Experience working with chronic, complex, and/or behavioral health populations (preferred).
