Clinical Social Worker

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of high-risk patients facing social complexities.

• Perform psychosocial assessments both in-person and via telephone with patients and/or their caregivers.

• Identify psychological and social barriers, creating suitable interventions to address care gaps and needs.

• Offer short-term counseling services when necessary.

• Coordinate services to ensure that patients and families comprehend the treatment plan.

• Collaborate with the care team to enhance quality and patient experience outcomes, including active participation in High-Risk Huddles.

• Reach out to community-based services to assist patients in aging at home.

• Establish and implement shared care goals with patients and their families.

• Partner with community-based organizations to build relationships and provide resources that promote patient independence.

⛳️ Requirements

• A Master’s degree in social work is required.

• Must hold a license as an LCSW or LISW, depending on the state.

• A valid driver’s license and a clean driving record are necessary.

• Ability to exercise sound clinical judgment and communicate effectively in both written and verbal forms.

• Strong computer skills are essential.

• Must be self-motivated and able to communicate effectively with professionals across various disciplines and programs.

• Capable of managing time efficiently, balancing multiple tasks, working within tight deadlines, resolving issues, identifying patient service trends, determining system improvements, and implementing changes.

• Ability to share expertise with others while fostering performance improvement to achieve patient satisfaction and efficiency.

• Experience in Care Management within a community-based setting or equivalent is required.

• Experience working with chronic, complex, and/or behavioral health populations is preferred.
