Clinical Social Worker

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of high-risk patients with socially complex needs.

• Perform face-to-face and telephonic psychosocial evaluations with patients and/or their caregivers.

• Examine psychological and social determinant barriers, and create interventions to address gaps in care and needs.

• Offer short-term counseling services when necessary.

• Coordinate services to ensure that patients and their families comprehend treatment plans.

• Collaborate with the care team to meet quality and patient experience goals, including participation in High-Risk Huddles.

• Reach out to community-based services that enhance patients’ ability to age in place.

• Establish and execute shared care goals with patients and their families.

• Partner with community-based organizations to cultivate relationships and provide resources that support patient independence.

⛳️ Requirements

• A Master’s degree in social work is required.

• Must hold a valid license as an LCSW or LISW, depending on state regulations.

• A valid driver’s license and a clean driving record are essential.

• Proficient in using sound clinical judgment and communicating effectively in both written and verbal forms.

• Above-average computer proficiency is required.

• Must be self-motivated and able to communicate efficiently with professional staff across various disciplines and programs.

• Skill in effectively planning time, managing multiple tasks, adhering to strict timelines, resolving issues, identifying patient service trends, improving systems, and implementing changes is necessary.

• Ability to share knowledge with others and an understanding of the importance of fostering performance improvement while ensuring patient satisfaction and efficiency.

• Experience in Care Management within a community-based setting or a similar environment is preferred.

• Prior experience working with chronic, complex, and/or behavioral health populations is preferred.
