This is a fully remote position, open to applicants in Nevada.
• Lead a team of Therapists and Care Extenders in executing programs, assisting members, and achieving outcomes for health plan partners.
• Collaborate with Therapists and Care Extenders to enhance their skills and provide direct support to members in difficult circumstances.
• Develop individualized service plans utilizing member screenings, discussions, data analysis, and knowledge of community resources.
• Assist members in addressing challenges and triggers, promoting relapse prevention, and minimizing risk.
• Motivate members to enhance their work skills and engage in therapeutic, social, and recreational activities.
• Inform members about mental and physical health issues, abuse prevention, violence awareness, medication management, and available community resources.
• Track, assess, and document member progress towards defined goals.
• Ensure accountability by communicating about upcoming appointments.
• Collaborate with members, families, physicians, and other providers to optimize resource utilization.
• Report significant incidents and issues related to quality of care.
• Develop communication strategies to engage the member population and support their objectives.
• Evaluate risk and conduct crisis intervention in line with safety protocols.
• Create and manage community resource guides focusing on medical health, behavioral health, violence prevention, and social services.
• Verify that member diagnoses and service plans are thorough and suitable.
• Lead CareMind teams to evaluate member needs, sustain care plans, and assist in achieving program completion.
• Organize caseloads and referrals, ensuring team schedules address program requirements.
• Participate in the recruitment process by screening, interviewing, and hiring team members.
• Provide orientation and training, conduct performance evaluations, and offer ongoing coaching to the team.
• Drive enrollment activities to develop new markets or sustain existing cohorts.
• Ensure the team meets essential performance indicators and achieves outcomes for members and health plan partners.
• Review documentation for quality assurance and training purposes.
• Analyze team performance, identify areas for improvement, and develop enhancement plans.
• Gather and analyze member outcome data to support training, market insights, partner relations, and results.
• Facilitate internal case review sessions and engage in external case management discussions with health plan partners.
• Maintain program-related and administrative data, reports, statistical records, and other necessary data collections.
• Collaborate with billing to assess claims for accurate service representation.
• A Master's degree in behavioral health, social work, or health care management from an accredited institution is required.
• An independent clinical behavioral health license in the state of Nevada is mandatory.
• Acceptable licenses include Licensed Clinical Social Worker (LCSW), Clinical Professional Counselor (CPC), Licensed Marriage and Family Therapist (LMFT), and Licensed Psychologist.
• Proven experience in leading, developing, and implementing programs while delivering outcomes.
• Prior experience in Health Coaching or Medical Social Work is preferred.
• Familiarity with suicide prevention and crisis management for adolescents, young adults, and individuals with severe mental illness is preferred.
• Experience in managing interdisciplinary care teams is advantageous.
• Knowledge of delivering care in compliance with applicable laws, regulations, policies, and procedures is essential.
• Ability to build, nurture, and enhance relationships and partnerships.
• Proven track record of identifying needs and taking independent action to drive change.
• Strong planning, organizational, time management, and prioritization skills to manage multiple tasks effectively.
• Exceptional listening, verbal, and written communication abilities.
• Capability to communicate information clearly and accurately to staff, business partners, stakeholders, and clients.
• Ability to collaborate effectively with others to establish goals, resolve issues, and make decisions.
• Competence in providing case/care management in both routine and crisis situations.
