Clinical Pharmacist – Oncology

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Take charge of medication management for designated patients.

• Assess and monitor medication therapy for suitability based on patient health conditions, institutional guidelines, and cost-effective practices.

• Conduct medication reconciliation, monitor medications, and provide education to patients.

• Instruct Pharmacy and Medical Residents as well as healthcare professionals.

• Contribute pharmacy expertise to order sets, benefit design, formularies, and pharmacy-related documentation.

• Offer pharmacokinetic consultation services.

• Collect and utilize laboratory data to assess drug selection and dosing.

• Relay findings to healthcare providers.

• Analyze patient responses to drug therapy.

• Document interventions and perform evaluations/outcome studies of clinical activities.

• Assist in the creation of pathways, best practices, and guidelines for specific patient populations.

• Educate staff, students, patients, healthcare providers, and other healthcare professionals.

• Submit work for publication every two years.

• Mentor residents, students, and new personnel.

• Support comprehensive patient care throughout all transitions beyond the hospital visit.

• Undertake both technical and non-technical tasks during staffing shortages, inclement weather, power outages, computer downtimes, or other emergencies.

• Adjust behavior and communication style to cater to specific patient populations while safeguarding patient privacy.

• Report any known or suspected unethical, questionable, safety, privacy, or compliance-related issues.

⛳️ Requirements

• Bachelor of Science in Pharmacy.

• Completion of a PGY1 Pharmacy Practice Residency, or three years of relevant specialty experience accepted in place of PGY1 Residency.

• Valid Pharmacy license in the state of practice.

• Previous experience in Oncology is preferred.

• Necessary computer skills and dexterity for data entry and retrieval of patient information.

• Proficiency with Windows-style applications and keyboard usage.

• Strong verbal and written communication skills.

• Capacity to present information clearly and professionally to various individuals throughout the patient care continuum.

• Excellent leadership and teaching abilities.

• Capability to work in a healthcare environment with possible exposure to hazardous medications or waste.

• Ability to frequently exert 10–20 pounds of force and occasionally up to 100 pounds of force.

• Doctor of Pharmacy (PharmD) is preferred.

• Completion of a PGY2 Specialty Pharmacy Residency is also preferred.
