Ambulatory Clinical Pharmacist

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Deliver integrated clinical pharmacy assistance to patients and healthcare providers through tailored relationships.

• Enhance medication adherence and advocate for evidence-based healthcare practices.

• Administer medication management, reconciliation, dose adjustment/optimization, laboratory test ordering, and adherence counseling in accordance with approved protocols/CPA/CDTM.

• Collaborate with interdisciplinary teams to create personalized care plans.

• Oversee medication regimens to ensure therapeutic objectives, monitor for adverse effects, drug interactions, and clinical criteria.

• Recognize opportunities for drug therapy enhancement and cost savings on medications.

• Conduct in-person and virtual assessments of disease states and provide drug therapy education.

• Engage in clinical outcome studies, REMS compliance/reporting, and initiatives for process improvement.

• Offer clinical justifications and supporting literature for prior authorizations and appeals.

• Coordinate medication management alongside physicians, specialists, and onsite pharmacists.

• Assist with peer-to-peer requests and targeted medication evaluations.

• Ensure adherence to FDA guidelines, clinical standards, and evidence-based literature.

• Streamline workflows and collaborate on operational modifications.

• Foster patient safety, medication literacy, education, and counseling.

• Uphold the confidentiality of organizational and patient information.

• Engage in professional education, publications, organizational activities, meetings, conferences, and developmental efforts.

• Carry out additional responsibilities as assigned by the Senior Director of Clinical Services and Manager of Clinical Services.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited pharmacy institution.

• Active pharmacist license in Colorado.

• One of the following: certification in a relevant practice area; completion of an accredited postgraduate residency with a minimum of 50% direct patient care experience within interdisciplinary teams; or at least one year of clinical patient services under a collaborative practice agreement/protocol, collaborative drug therapy management, hospital pharmacy practice, or healthcare system.

• Fulfill continuing education requirements to maintain Colorado pharmacist licensure if selected.

• Ability to analyze data and demonstrate sound judgment.

• Excellent verbal communication abilities.

• Comprehensive knowledge of pharmacology advancements and FDA health regulations.

• Strong technical and clinical competencies.

• Commitment to legal compliance, productivity, quality, and meticulous attention to detail.

• Profound clinical knowledge and confirmed expertise in disease-state management and treatment protocols.

• Well-organized, trustworthy, dependable, empathetic, and discreet.

• Proficient computer skills, including Microsoft Outlook, Teams, Word, Excel, PowerPoint, internet usage, and web-based platforms.

• Data entry and computer skills will be assessed during the pre-employment evaluation.
