Clinical Case Manager, Behavioral Health

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Perform thorough behavioral and physical health evaluations utilizing clinical tools and data analysis.

• Create and manage personalized care plans that align with member benefits and available resources.

• Collaborate with healthcare providers and internal teams to ensure efficient and cost-effective care delivery.

• Offer crisis intervention and follow-up support for members facing behavioral health or medical emergencies.

• Employ Motivational Interviewing and advanced clinical judgment to mitigate risk factors and enhance outcomes.

• Organize psychosocial wraparound services and community resources.

• Assist members with intricate behavioral and physical health needs from their homes.

⛳️ Requirements

• A valid, unrestricted independent clinical license in your state (LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, or LMHC).

• A minimum of 3 years of direct clinical experience following a master’s degree in a hospital, outpatient, ambulatory, or similar environment.

• At least 3 years of experience dealing with mental health and substance use disorders.

• Ability to provide care over the phone while documenting in real time.

• Proficient in Motivational Interviewing techniques.

• Strong technological skills.

• Master’s degree in Social Work or Counseling.

• Experience in crisis intervention is preferred.

• Background in managed care or utilization review is preferred.

• Experience in case management and discharge planning is preferred.
