Certified Community Health Worker, CHW

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct community outreach to assist patients facing complex medical and social challenges.

• Support patients in navigating the healthcare system effectively.

• Onboard and evaluate new Community Health Workers.

• Assess staff proficiency during their orientation and provide necessary training support or direction.

• Identify and engage with patients who have intricate medical needs.

• Collaborate with a multidisciplinary team to create and implement comprehensive care plans.

• Interact with patients through phone calls, home visits, hospital visits, and accompany them to provider appointments as needed.

• Measure and document vital signs.

• Recognize care gaps and connect members with appropriate resources.

• Facilitate telemedicine appointments between members and providers.

• Deliver disease-specific educational resources.

• Conduct medication reviews to ensure adherence to prescribed regimens.

• Perform safety evaluations in patients' homes.

• Enhance adherence to treatment plans by identifying and resolving care gaps proactively.

• Communicate any concerns to the clinical team and manager, seeking assistance and guidance as needed.

• Participate in multidisciplinary team meetings to optimize treatment plans.

• Reinforce treatment plans under the supervision of the Case Manager and Licensed Social Worker.

• Work remotely while engaging in extensive travel throughout designated Pennsylvania communities and counties.

⛳️ Requirements

• Candidates must be residents of Pennsylvania.

• Reliable transportation is essential.

• Willingness to travel frequently within the designated coverage area.

• Certification as a Certified Community Health Worker (CCHW) from the Pennsylvania Certification Board; certification must be submitted with the resume.

• High School Diploma or equivalent (GED) is required.

• At least 1 year of relevant work experience is necessary.

• Skills required: Clerical I, Communication, Multitasking, Problem Solving.
