Case Manager – Specialty Case Management

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Create an Individual Plan of Service utilizing person-centered planning methodologies.

• Continuously link, coordinate, and monitor services provided to clients.

• Travel throughout the state of Michigan, including overnight stays, as required by the role.

• Maintain a work schedule from Monday to Friday, 8:00 am to 5:00 pm.

• Successfully perform essential duties while assisting individuals with mental illness, substance use disorders, and intellectual or developmental disabilities.

⛳️ Requirements

• A Bachelor’s degree in a human services-related discipline such as Social Work, Education, Psychology, or Sociology.

• A minimum of six months of full-time experience working with individuals facing mental illness, substance use disorders, or intellectual/developmental disabilities; relevant fieldwork or internship experience is applicable.

• At least one year of experience in treating or working with individuals with intellectual disabilities, or specialized training obtained through fieldwork and/or internships.

• A valid driver's license and access to dependable transportation for job-related activities.

• Strong organizational capabilities and familiarity with person-centered planning.

• Proven understanding of individuals with severe mental illnesses and substance use disorders.

• Some roles may require a demonstrated understanding of individuals with intellectual or developmental disabilities.

• Certain positions necessitate an understanding of individuals and significant behaviors present in a residential context.

• Effective communication skills that convey information clearly.

• Proficient active listening skills, including the ability to listen attentively and ask relevant questions.

• Strong time management abilities.

• Successful completion of a drug screening as part of the background verification process.

• Capacity to perform each essential duty competently.
